SMHC recognizes the unique needs of patients who aren’t well enough to be discharged (as their insurance may require), but who no longer need more expensive, acute inpatient care. Our Sub-Acute Care Center provides the solution: an appropriate level of treatment and monitoring, for a suitable length of time, at a significantly lower cost.
The transitional phase between acute care and home averages 10 to 15 days. During that time, patients are cared for in a comfortable, home-like setting with 24 beds, a dining facility, recreation and lounge spaces, and a rehabilitation suite.
Every handicapped-accessible room is beautifully decorated and equipped with an adjustable bed, private bath, telephone and television. And every patient commands the attention of our fully certified staff.
Most sub-acute patients need a greater level of rehabilitative care and frequent assessment of a treatment plan. Typical services provided by our Sub-Acute Care Center include:

Staff
The Sub-Acute Care Center uses an interdisciplinary team approach that allows for holistic patient care, which treats the whole person not only for illness, but also for emotional and social effects of illness. The staff is directed by a Licensed Nursing Home Administrator and includes:
Medical Director– specializing in Geriatric and Internal Medicine Full-time Psychiatrist– specializing in Rehabilitative Medicine Consulting Physicians – available to address dental, podiatric, auditory, ophthalmic and other needs Physical and Occupational Therapists – available six days a week Speech Therapist– available as needed Licensed Clinical Social Worker– assisting in psychosocial needs, community resource education and discharge planning Certified Recreation Therapist– providing structured activity programs, leisure counseling and one-to-one interaction Registered Dietician– specializing in dietary needs of adult and elderly populations Beautician– available by appointment
Our staff also includes a full complement of RN’s, LPN’s and Certified Nursing Assistants – backed by all of Southern Maryland Hospital Center’s resources – to deliver the extra special care our patients need after surgery or other trauma caused by illness.

SMHC’s Sub-Acute Care Center staff is committed to treating each patient with dignity and compassion, and returning them to their highest level of functioning.
We serve all adult residents of Southern Maryland and surrounding areas who may benefit from rehabilitative or complex medical management. The Sub-Acute Care Center does not make “any preference, limitation or discrimination based on race, color, religion, sex, handicap, familial status or national origin, or an intention to make any such preference, limitation or discrimination.”
The Sub-Acute Care Center is licensed and certified by the State of Maryland and the Health Care Financing Administration (HCFA) to provide skilled nursing services under Medicare, Medicaid, and many Managed Care Programs. It is fully accredited by the Joint Commission on Accreditation of Healthcare Organizations.
