HEALTH CARE BENEFIT PLANS


 Who is Eligible?

 

  1. All active employees
  2. Work at least 20 hours per week
  3. Eligible the first of the month following 60 days of active employment
  4. Eligible dependents include:
    1. Your spouse, unless legally separated
    2. Your children, up to 26 years of age who are primarily dependent upon you for maintenance and support, and are not regularly employed on a full-time basis.  Dependent children include natural children, stepchildren, or legally adopted children.

Medical Care Benefits

 

 

  1. CareFirst Blue Choice – Health Maintenance Organization (HMO)
    1. Managed Care Plan:  requires selection of a Primary Care Physician
    2. Referral is needed to see a specialty care participating Physician
    3. Preventive services – no charges and no deductibles apply
    4. $250 deductible per person, maxes out at $500 per family
    5. Co-pays are $20.00 for PCP, $40.00 for Specialist
    6. Inpatient care must be pre-certified by CareFirst

 

  1. CareFirst Blue Choice - Open Access is designed to allow a little more flexibility

a.       Out-of-Network Benefits – no referrals required

b.      $250 deductible per person, maxes out at $500 per family

c.       Preventive services – no charges and no deductibles apply

d.      Co-pays are $20.00 for PCP, $40.00 for Specialist in network

e.       Out-of-network, after meeting the deductible, the patient is responsible for 20% of bill and any expenses that are not covered by healthcare plan

 

 

 Prescription Drug Coverage - provided through CareFirst Blue Cross

  1. $100 deductible per family member
    1. After meeting the deductible, co-payments for prescription and refills
  2. $20 - $50 co-payment pharmacy charge for injectibles
  3. One co-pay per 31 days consecutive supply for retail drugs
  4. Two co-pays for 90 day supply for mail order drugs or retail pharmacy drugs
  5. Oral contraceptives have three co-payments for a 3-cycle supply

 

 

LIFE, SHORT-TERM DISABILITY, AND LONG TERM DISABILITY INSURANCE BENEFITS

 

Short Term Disability is an income replacement plan in the event of an illness or accident occurrence outside of work.

  1. Eligible the first of the month following 60 days of active employment
  2. Offered to full-time employees
  3. 60% of salary paid up to $750/$1000 weekly
  4. Elimination periods are 8 days, 15 days or 31 days
  5. Employee paid benefit

  

Long Term Disability is an income replacement plan in the event of an illness or accident occurrence outside of work. Long Term Disability is received 180 days from the beginning of illness or accident.

1.      Automatically enrolled after 12 months of continuous full time employment

2.      Premium is paid by Southern Maryland Hospital

3.      Employee pays an administrative fee of $0.25 per pay period

 

Basic Life and AD&D 

 

Southern Maryland Hospital provides basic life and Accidental Dismemberment and Disability insurance to all full and part-time employees who work 40 to 80 hours per pay period.

  1. Eligible the first of the month following 60 days of active employment
  2. Premium is paid by Southern Maryland Hospital
  3. Coverage is 1 times your annual salary, max. $100,000

 

Supplemental Voluntary Life Insurance 

 

Southern Maryland Hospital offers at the employees’ expense additional supplemental voluntary life insurance to all full and part-time employees who work 40 to 80 hours per pay period. 

  1. Eligible the first of the month following 60 days of active employment
  2. Employee must contact the Benefits Department for enrollment
  3. Maximum guaranteed issue is $200,000
  4. Maximum guaranteed issue for children is $10,000
  5. Premium is paid by employee by payroll deduction
  6. Amounts of coverage can be increased at annual open enrollment

 

 

RETIREMENT BENEFIT

 

401(k) Retirement Savings Plan 

 

Southern Maryland Hospital offers its employees the opportunity to contribute to a Tax Sheltered Retirement Savings Plan – 401(k). 

  1. Employees can contribute up to $16,500 of pay. 
  2. Employees age 50 and over can contribute an additional “catch up” of $5,500 for the year 2011. 
  3. Southern Maryland Hospital will match 50% of the employee’s contribution up to the first 4%. 
  4. Enrollment into the 401(k) retirement savings plan is during the months of January, April, July, and October of each year following 90 days of employment.

 

 

OTHER BENEFITS

 

Flexible Spending Accounts 

 

The flexible spending is offered to full and part-time employees who work 40 – 80 hours per pay period.  The plan allows employees to allocate pre-tax dollars to pay for uncovered medical, dental, vision, and some over-the-counter medications, and for dependent care expenses.

 

 

Training  

 

Seminars and training is a benefit offered to full and part-time employees working 40 – 80 hours per pay period.  Eligibility is immediate.

 

 

Tuition Assistance Plan 

 

Tuition assistance is paid upon successful completion of a pre-approved course up to an annual maximum of $3,000 for undergraduate and $4,000 for graduate school.  Tuition assistance is available to all full-time employees.  Eligibility for Tuition Assistance is allowable after successful completion of the 90 day probationary period.

 

 

Paid Time Off (PTO) 

 

Southern Maryland Hospital employees receive time off through the hospital’s “Paid Time Off” (PTO) plan.  This plan combines the employees’ vacation, sick, personal business, holiday leave into one account based on years of service.
