Registered Nurse

  • Carries out physician orders
  • Assesses patient medical status
  • Teaches about medications and special diets
  • Teaches about various disease processes
  • Performs treatments

Physical Therapist

  • Evaluates patient status
  • Develops exercise programs tailored to patient needs and abilities for gross motor skills
  • Promotes independence with activities of daily living
  • Teaches proper use of equipment


Occupational Therapist

  • Evaluates patient status
  • Develops exercise program tailored to patient needs and abilities for fine motor skills
  • Promotes independence with activities of daily living
  • teaches how to safely perform routine day-to-day activities


Speech Therapist

  • Evaluates patient status
  • Develops exercise programs to assist with improving communication skills
  • Develops exercise programs to improve swallowing ability
  • Develops program to improve cognitive skills


Medical Social Worker

  • Assists with arranging for patient needs such as housing, food, caretaking and finances
  • Assists with referrals to community resources


Home Health Aide

  • helps with personal care
  • Assists with activities of daily living
  • Performs light housekeeping incidental to the patients needs
  • Performs other activities under the direction and supervision of a registered nurse

SMHHS is state licensed and Medicare certified. We cover all aspects of health care including Infusion Therapy.
